PHYSICAL THERAPY

Initial Visit:

On your first visit the Physical Therapist will evaluate, treat, and discuss your plan of care with you.  Please do not hesitate to ASK QUESTIONS that pertain to your actual treatment or related matters.  Our expertise combined with your effort will help to reach optimal results in a timely fashion. 

 

Referral:

A physician’s referral is preferred, but not required before starting physical therapy. Many physicians work closely with our practice. Regular typed P.T. evals and progress reports are sent to your referring Physician. We accept referrals from Medical and Osteopathic Physicians, Podiatrists, and Dentists.

 

Scheduling:

When calling to make an appointment please give the following information:

 

• Your name and phone number

• Physician’s name

• Your diagnosis or type of injury

• Insurance authorization; Information

 

Please bring your referral with you at the time of your first visit.
